INDIANA Statute: Ind. Code Ann. 16-37-2-10(b) (2004)
(providing for change of birth certificates)
Indiana will issue a new birth certificate with your new name. (Indiana birth certificates do not designate a person’s sex).
You will need a copy of your old birth certificate and a copy of the court order for your name change, along with fees of $5.00 for the procedure and $5.00 for each additional certificate you request.
Contact the health department of the county in which you were born.
There is no statute, case, administrative code, or other government document that addresses the issue of amending a birth certificate to reflect sex reassignment. Our practice of making such amendments rest solely upon the force of a court order specifically directing a change to the birth certificate.

(1) For a person born in this Sate, the State Registrar of Vital Records shall establish a new certificate of birth when he receives any of the following…(d) An affidavit by a physician that he has performed an operation on a person, and that by reason of the operation the sex designation on such person’s birth record should be changed. The State Registrar of Vital Records may make any investigation or require any further information he deems necessary.
